Mission Statement

At Central ENT Consultants, our mission is to provide comprehensive Otolaryngology, Head and Neck Surgery, Sinus Sleep and Allergy Services. In addition, provide audiologic services, including hearing aid services and repair, through our audiologists on site.

We ask that you please arrive 20 minutes prior to your scheduled appointment to allow time for your new patient information to be completed. Please print and fill out the following forms to bring with you so that we may expedite your first visit:

Patient Portal

With our records being fully electronic, we are able to provide our patients with access to a Patient Portal that will allow you to have access to your health information. By providing our office with an email address, we can send a link directly to your inbox. Otheriwse, we have placed a link to the Patient Portal here and on our home page. We can provide you with a unique log in ID and password to access this. Please call our office to obtain the temporary password as well as your login.

What to bring

Having everything you need for your appointment will ensure that you are able to provide us with the most accurate and up to date information that you have. Please bring your ID and Insurance Card for your first visit as well as any necessary co-pays. Please have your paperwork complete prior to your appointment time as well. If there are any documents from other office visits that will help the doctor better diagnose your issue, please pring those along with you as well.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
